File an Annual Pesticide Report

DEC encourages you to file your report electronically. This method of reporting may be the least costly to you, while being the most accurate method of reporting. Please visit the pesticide electronic reporting website for more information.

The recordkeeping example shown in the Important Links section on the right of this page includes samples of ideal recordkeeping information. Please use this as a guide when preparing annual reports and daily use records.

For individuals who wish to hand-write their report, blank pesticide reporting forms that can be printed are available in the right hand column of this page under Important Links.

The PRL also reaffirms record keeping requirements for commercial permittees, commercial applicators/technicians and private applicators.

Records of the restricted pesticides purchased and used by a certified private applicator need to be maintained on the "Certified Private Applicator Record of Restricted Pesticide Purchase and Use" form. These forms are not submitted to the DEC, but maintained on site for at least three years. To obtain this form, contact us via email or telephone using the information on the right.

DEC has pads of the "Applicator/Technician Pesticide Annual Report." These pads are provided free to the regulated community. For convenience, the back of each page provides an area for recording daily record keeping information (Dosage Rate, Method of Application, Target Organism, Place of Application). Order pads of Form 44-15-26. Handwritten reports and electronic media should be mailed to: NYSDEC, Pesticide Reporting, PO Box 10699, Albany, NY 12201-5699.
