The applicant proposes to construct one new 20 inch diameter, 1000 gallons per minute well, approximately 635 feet deep at Plant No. 3.  The new well will operate in conjunction with existing Well No. 3-2 to provide additional capacity.  The site is located at Jerusalem Ave. and Chain La., Hicksville, Town of Oyster Bay.

Applicant proposes to construct 50 linear feet of new bulkhead a maximum of 18' seaward of average high water, with a 23' return on north end and a 10' return on south end, and dredge 5' seaward along  the new bulkhead to -4' mean low water. The 25 cubic yards of dredged material is proposed to be placed behind the new bulkhead, filling below mean high water, to bring the area up to grade for proposed parking. Additionally, applicant proposes 6 new mooring piles along bulkhead for head in mooring of 5 boats. Project is located on East Shore Road, Lindenhurst.

The applicant proposes a discharge of 26,100 gallons per day of laundry wastewater to groundwater from an LRF 4AC Alltec filtration water treatment system at the applicant's facility located at 875 Horseblock Road, Farmingville, Brookhaven Town, Suffolk County.   The applicant plans to continue to operate a laundromat.

The Department has made a tentative determination to issue a permit for a State Pollutant Discharge Elimination System permit under Article 17 of the Environmental Conservation Law.  This tentative determination indicates that the discharge regulated under this statute is considered to satisfy regulatory standards for permit issuance, based on information available to the Department at this time.  As a tentative determination, the Department seeks comments on the proposed regulated activity prior to making a final decision on permit issuance.  As such, this tentative determination does not signify department approval for, or endorsement of, the activity for which a permit is sought or for the overall proposed project.

Refer to this application by application number listed above and SPDES number,  NY 021 0919.  The proposed discharge is located within an area designated as a sole source aquifer.  The following water purveyors have been identified as having service areas located within a 3mile radius of the applicant's facility: Suffolk County Water Authority.

The applicant proposes to construct a dwelling and attached deck 50 feet from the boundary of state regulated freshwater wetland # NA-6.   The applicant also proposes to install a pervious driveway 7.5 feet from the freshwater wetland boundary, as well as a drywell 43 feet from the freshwater wetland boundary, and an inground pool 62 feet from that boundary.  Ground disturbance, associated with the installation of the driveway, is proposed within 6 feet of the freshwater wetland boundary.  The proposed septic system will be located greater than 100 feet from the freshwater wetland boundary, and therefore outside the jurisdiction of Article 24 of the NYS Environmental Conservation Law.  The project is located at 17 Whalers Lane, in Amagansett, Town of East Hampton, SCTM # 300-152-215.024.

The applicant proposes the construction of a new 16" diameter, 130' deep well which will have a pump capacity of 1,000 gpm.  This well will serve as a water supply well for the Hampton Bays Water District.  The well ( No. 5-1) will be located at the Jones Road Plant, just south of  Montauk Highway, east of Jones Road, Hampton Bays, Town of Southampton .  ( SCTM #900-253-2-1)   The entrance to the plant will be located south of the property from Bellow Terrace.

Applicant proposes to resubdivide an approximately 8.745 acre parcel of agricultural land with frontage on Mecox Bay. This land currently exists as five lots (1.6 acres, 3.3 acres, 1.1 acres, 1.0 acres & 1.3 acres, respectively) created by a 1928 old filed map. The proposal will reduce the total number of lots from five to three. The three proposed lots will be 134,040 square feet, 122,796 square feet and 124,116 square feet in area, respectively, and will conform with all municipal zoning requirements. The site is located on the east side of Crescent Avenue, south of Halsey Lane in Water Mill. Current SCTM #s 0900-115-2-lots 30.2, 30.3, 30.4, 30.5 & 30.6.

Applicant proposes to subdivide 24,727 sq. ft. parcel into two parcels,  (1)12,124 sq. ft. parcel to be combined with existing 23,379 sq. ft.adjacent parcel to the north for a final parcel size of 35,503 sq. ft., and (2) 12,603 sq. ft. parcel to be combined with existing 24,772 sq. ft. adjacent parcel to the south for a final parcel size of 37,375 sq. ft. Project is located on Main Bayview Road, Southold.
