The Department has received an Air State
Facility application from Callanan Industries
Inc, DBA Iroquois Rock Products, for existing operations at its Henrietta Hot Mix Asphalt
Plant, located at 60 Ridgeland Road in the Town of Henrietta, Monroe County.
As part of this application, Callanan Industries
has proposed to continue to restrict the
Henrietta Hot Mix Asphalt Plant potentials to emit carbon monoxide (CO), oxides of
nitrogen (NOx), sulfur dioxide (SO2) and volatile organic compounds (VOC) to below the
6 NYCRR Part 201-6 (Title V) and Part 212 Reasonably Available Control Technology
(RACT) applicability thresholds for a Major Stationary Source.
In response, the Department has drafted
a Special Permit Condition to limit asphalt
production in any 12-month period sufficiently to restrict CO emissions to less than 100
tons. This asphalt production limit will also reduce facility potentials to emit NOx, SO2
and VOC to less than the CO emissions, below the Part 201-6 and Part 212 thresholds
of 100 tons per year each for CO, NOx and SO2, and 50 tons per year for VOC.
To verify that the facility is in compliance
with these permit conditions, Callanan
Industries will be required to maintain a log of monthly asphalt production at the facility,
12-month rolling totals of asphalt production, and calculate annual totals of emissions.
These records are to be maintained for a minimum of five years, and must be made
available with all supporting data to Department inspectors upon request.
Also as part of this application, Callanan
Industries has proposed to use waste oil,
classified as "Waste Fuel A" in 6 NYCRR Part 225, as a fuel, in addition to natural gas
and #2 fuel oil. Waste Fuel A does not contain chemical waste and is subject to
contaminant limitations specified in Part 225-2.4(b). For all three fuels, the contaminant
emitted at the greatest rate remains CO.

Applicant proposes to make
modifications to the William D. Denise Reservoir (a.k.a.
Southeast Reservoir) an existing public water supply reservoir with a maximum capacity
of 55 million gallons, which is located on the west side of West Bloomfield Road at
intersection of Thornell Road in the Town of Pittsford. The modifications include
installation of a floating cover, improving site security facilities, and constructing
stormwater management improvements on-site. Minor reconstruction will occur at the top
of the existing reservoir embankment to allow installation of a new concrete anchor wall
for the floating cover, shallow burial of stormwater piping down sideslopes and
reconstruction of existing overflow. The proposed spillway will exceed the capacity of
the existing spillway, providing sufficient weir length for the maximum inflow to be
passed and accommodating storage of 150% of the 100-year storm event or 50% of the
24-hour Probable Maximum Precipitation Event. A Dam Safety Permit will be required
before work can begin on these modifications to the reservoir, which is classified as a
New York State Large, Class B embankment dam.

The applicant proposes to
install an aluminum staircase from the top of the bank/bluff to
the beach in order to access Lake Ontario along his property on Lake Road in the Town of
Webster, Monroe County. The proposed staircase will be 36 inches wide and will include
two 40 foot long staircases. The staircase will be anchored to a 4 foot by 4 foot platform
at the top of the bank and a 40 inch by 8 foot platform will connect the two staircases in
the center. There will be no heavy equipment used on the the slope and staked straw
bales will be install between the post hole digging and the lake.

The Department has received
an application for the installation of septic tank, sand bed,
aerator and chlorination system to serve the Mill Creek Golf Course Clubhouse located at
128 Cedars Avenue in the Town of Wheatland. The applicant proposes to discharge
3,308 gallons of treated sanitary and kitchen wastewater to a drainage ditch which flows
to Mill Creek, a class C(t) stream. The permit contains seasonal effluent limits for
biological oxygen demand, suspended solids, settleable solids, pH, ammonia, dissolved
oxygen, and fecal coliform. The Department has made a tentative determination to
approve the application. As a tentative determination, the Department seeks comments
on the proposed regulated activity prior to making a final decision on permit issuance. A
draft permit is available for review.
