construction of a 14.6 foot high earthen dam with service and
auxiliary spillways on their property located on the east side of Markhams Road
approximately one mile west of its intersection with Wolf Road. The dam would
impound 14.38 acre feet of water with a surface area of 3.25 acres and has been
determined to be a Hazard Class "A" structure. The application materials are available for
review and comment in the regional office during normal business hours.

The Town of Carroll requests
approval of its Frewsburg Water District Extension No. 10,
which includes the extension of total of 11,400 linear feet of water mains along Warren
Rd., Riverside Rd., and Frew Run Rd., and the taking of a supply of water in amounts
estimated to average 22,500 gallons per day to serve approximately 150 customers
through 50 service connections. Maximum daily demand is estimated at 56,250 gallons
per day. Water is to be supplied by the water district's existing approved wells.

2.7 acre addition to the Merle Goot Gravel Mine, previously
approved for 16.68 acres. The proposal includes the addtion of 2.2 acres on the Betts'
property, which is located contiguous with and west of the current mine and 0.5 acre by
the elimination of the 25 foot setback on the Tri-James property at the Betts' property
line. No changes in the mine operating procedures or reclamation objective are proposed.
The Merle Goot Gravel Mine is located 1,200 feet north of Barnard Road and 1,000 feet
east of Barnum Road, in the Town of Charlotte, Chautauqua County.

install tree revetments along 250 feet of severely eroded creek
bank, create a heavily-vegetated floodplain bench, realign approximately 225 feet of
creek channel and create trout habitat. Additionally, an existing dirt access road will be
moved away from the creek to make room for the floodplain bench construction. The
project site is located along Wiscoy Creek and within the regulated 100-foot wide
adjacent area of Freshwater Wetland PI-4 in the Village of Pike on the west side of Main
Street at the Wyoming County Fairgrounds.
