construct a 560 foot long by 12 foot wide driveway, of which
50 feet will be within Freshwater Wetland BN-14 and approximately 350 feet will be in the
adjacent area. The driveway will serve a single family residence. Construction will consist
of installation of a 5 foot diameter culvert and approximately 300 yards of gravel for the
driveway. The site is located on Camp Road, approximately 1/4 mile east of its
intersection with State Route 80.
