The applicant proposes to mine bluestone from a 58 acre mine on located on a 166 acre property owned by the applicant.  30 acres are proposed to be affected during the initial 5 year permit term.  Approximately 3 - 4 million tons of stone is proposed to be mined over the entire life of mine.  The operation will include blasting, sawing and crushing.  All top soils and subsoils will remain on site.  The site will be reclaimed as vacant grassland.

The applicant is proposing the operation of a Part 360 transfer station and storage facility which would allow the facility to accept the return of used syringes from medical facilities to which Pharmalogic delivers radiopharmaceuticals. The materials will be stored until the radioactive sharps have decayed to background levels. The sharps will then be transferred to a New York State licensed medical waster transporter for treatment and off-site disposal. The proposed approved design capacity of the facility is 500 pounds per month.  

The Department has made a tentative determination to approve this modification to relocate and reconstruct the outfall from the Nott Road Sewer Treatment Plant, to the north side of the confluence of the Hunger Kill and Normans Kill. Construction will include approximately 100 feet of rip-rap stream bank stabilization on the north shore and 100 feet of willow planting stream bank stabilization on the south shore.  Approximately 20' of the existing outfall pipe will be removed, and the end will be capped.  The SPDES permit modification is needed to identify the new outfall location, no changes to the effluent limits or monitoring requirements of the existing SPDES permit are proposed.

The applicant proposes to clearcut weeds/vegetation and lilly pads from a 30, 000 sq.ft. area of Lower Rhoda Pond (Lake Rhoda) using a portable underwater weed cutter.  The weeds are proposed to be cut in the north end of the lake around an existing small island which is the general beach area of Camp Pontiac an existing childrens camp.  Cut vegetation will be removed from the water body.  The site is located at the northern end of Lower Rhoda Pond, in the Town of Copake, Columbia County.

The applicant proposes to expand an existing 5.0 acre life-of-mine surface consolidated sandstone mine to 28.2 acres.  Mining will progress in 4 to 5 acre phases with concurrent reclamation.  A portable crusher may be used on-site for processing overburden or waste bluestone for off-site sale.  The mining operation will include blasting and the mechanical cutting of stone.  After cessation of the quarry operations, the quarry will be reclaimed to its previous land use as unmanaged natural habitat.  The project is located off County Route 20, opposite the intersection with Beals Road.

The applicant proposes to repair approximately 180 feet of severely eroded streambank of Gooseberry Creek, to protect a recreational bike path.  The bank will be stabilized by constructing a stone block wall approximately 6 feet high.  The slope behind the stacked wall will be backfilled, regraded, and vegetated.  The thalweg of the creek will be relocated to its original location away from the toe of the bank. The project is located off Bloomer Road, approximatley 1/2 mile northwest of the County Route 16 intersection.

The applicant proposes to conduct a stream bank stabilization project along approximately 1700 feet of the Stony Clove Creek to reduce water quality impacts from the eroding banks.  The project will utilize natural stream channel design concepts, rock grade control structures, and rock bank erosion control structures.  The project is located off State Route 214 adjacent to the Beecher Road intersection, near the hamlet of Lanesville.

The applicant proposes to construct a driveway over SR-203-1 and in Freshwater Wetland HW-26 and its adjacent area for access to a single family residence building site.  A bridge will be constructed to cross the stream. The project is located on the north side of County Route 11 approximately 1/2 mile east of the Petkewee Road junction.
