ENB Region 1 Completed Applications 04/16/2003

The applicant proposes to subdivide a 75,493 square lot  into two lots; Lot A will be 49,093 square feet and Lot B will be 26,400 square feet.   In addition, the applicant proposes to demolish the existing single family dwelling, garage and patio on Lot B. The project is located at 200 Sage Avenue, Lawrence, Town of Hempstead, Nassau County, NCTM#: Section 40, Block J, Lots 416, 418, 421, 424, 429 &1924.  

The applicant proposes to construct a new production well 490 feet deep, 20" x 10" diameter, with a capacity to pump approximately 1,300 gallons per minute.  The well will be part of the South Shore Low II water supply system which serves the south shore from the Connetquot  River in Oakland to Station Road in North Bellport.  The well site is located at China Road and Lincoln Avenue, Sayville, Town of Islip.

Subdivide 19,075 sq ft parcel into two lots, 9,325 sq ft and 9,750 sq ft .   Remove existing garage and construct new dwelling.  The new dwelling will be locatd approximately 45 linear feet from NYSDEC regulated Freshwater Wetland A-14 .     The parcel is located at 62 Overland Ave., Amityville, SCTM # 0100-168-01-14.     

The Department has made a tentative determination to modify the SPDES permit for an existing facility to increase the flow rate effluent discharge to 15,000 gallons per day of (laundromat) process wastewater into groundwater from two outfalls. The process wastewater will be treated by an Alltec system prior to discharge.  The facility is located at 2073 Middle Country Road, Centereach, Town of Brookhaven, Suffolk County (SPDES # NY 013 7243).  The existing discharge is located within an area designated as a sole source aquifer.  The following water purveyor has been identified as having service  areas located within a 3 mile radius of the applicant's facility: Suffolk County Water Authority.

This tentative determination indicates that the discharge regulated under this statute is considered to satisfy regulator standards for permit issuance based on information available to the Department at this time.  As a tentative determination, The Department seeks comments on the proposed regulated activity prior to making a final decision on permit issuance.  As such, this tentative determination does not signify department approval for or endorsement of the activity for which a permit is sought or for the overall proposed project.  Refer to this application by application number listed above and SPDES number NY0137243.  A copy of the application and draft permit are available at the Region I Stony Brook Office.

Applicant proposes to construct an 8' x 36' deck addition, a 6' x 9' deck addition, and a 2nd story 5' x 15' deck within 18 ft. of the tidal wetland boundary and has requested a variance from the required 75 ft. setback for structures in accordance with Part 661, Tidal Wetlands Land Use Regulations.  As mitigation, the applicant proposes to reduce the existing lot coverage by impervious surfaces by removing the 1256 sq. ft. asphalt driveway and replacing with pervious material (stone).  The property is located at 24 Sunset Lane, Patchogue, Town of Brookhaven, County of Suffolk on the Great South Bay.
