The applicant proposes to
install new bulkhead across the opening of an existing 34' x 14'
boat slip and then fill it. Additionally, the project entails the construction of two 3' wide
travel-hoist launch ramps, one on each side of the launch area, maintenance dredge the
16' x 35 launch area to a depth of 5' below mean low water, placing the resultant material
in the filled boat slip area and the construction of a new 2' x 20' catwalk along-side the
launch ramps. An 2' x 20' existing catwalk will be removed. The project is located at the
western end of Sumpwams Ave., along the eastern shore of Carlls River in the Village of
Babylon, SCTM # 0102-016-1-13-31.

for an existing facility to discharge 26,000 gallons per day of (laundromat) process
wastewater, process via an ALLTEC LRF-4AC wastewater treatment system, into
groundwater from one outfall. The facility is located at 146 Mastic Beach West, Shirley,
Town of Brookhaven, Suffolk COunty. The proposed discharge is located within an area
designated as a sole source aquifer. The following water purveyor has been identified as

The applicant proposes to
redirect stormwater runoff from the adjacent roadway away
from the foundation of the viaduct by installing a new concrete curb and gabion wall, and
clearing and grading to create a series of two retention pools and swale, adding topsoil
and installing plantings. In addition, the applicant proposes to install an oil/grit separator
in line with an existing 18" reinforced concrete pipe leading to the proposed retention
pools. The applicant also proposes to remove wood debris, timber walls and wood
fencing and install new chain link fencing and timber wall. The project is located at the
MTA/LIRR Smithtown viaduct, south side of West Main Street (State Route 25),
Smithtown, Town of Smithtown, Suffolk County.
