Project Description: The applicant proposes to
modify the 100 foot adjacent area of Freshwater Wetland WF-16 (Class 3) in conjunction with the construction
of an automobile service building on a 2
acre site located on the east side of US Route 9 immediately north of the Village of
Fishkill. Proposed work within the adjacent area includes placement of approximately
4,800 cubic yards of fill and construction of portions of a gabion retaining wall, rip-rap
slope and parking area. The total extent of disturbance within the regulated area is
approximately 16,200 square feet to accommodate 37 parking spaces and related
roadway.

Project Description: The Department has made
a tentative determination to approve this application to modify
a permit to add an additional outfall (Outfall No. 003) for a surface discharge of 2,800
gallons per day (gpd) of treated sanitary wastewater to a tributary of the Middle Mongaup
River (Class B) from the applicant's operating seasonal camp. The project will increase
the overall discharge from the camp to 24,550 gpd and involves the construction of a new
sand filter system with two septic tanks, a dosing tank with chlorination system, and an
outfall to a swale at the existing operating camp facility. Camp Machne Gila is located on
NYS Route 55 (Liberty Road), north of the intersection with Ferndale-Loomis Road in
the Town of Liberty, Sullivan County. A draft permit is available for inspection and
comment at the regional office.

Project Description: The Department has made
a tentative determination to approve this application for a
modification of a permit (SPDES ID# NY0250988) for a proposed discharge of 17,000
gallons per day of treated sanitary wastewater into the groundwater from a proposed
landspreading system to be constructed at the applicant's facility, Camp Shaary Chedva
Be'ikvei Hatzoin. The new landspreading system will replace the existing failing
subsurface leach system at the camp. The facility is located on the west side of Painter
Hill Road, Mountaindale, where the applicant operates a summer camp for religious
instruction.

Project Description: The Pine Hill Water Company
has applied for modification of its existing Public Water
Supply Approval permit (WSA No. 5889-filed August 31, 1970). The Company provides
water service to the Hamlet of Pine Hill. The purpose of this application is to correctly
document existing sources of supply owned by the Company and to establish system
demand flows and source capacities. Records kept by the Company indicate an average
of 144,273 gallons per day (gpd) through its treatment system during the period from
June 1, 2000 to January 23, 2001. Following system leak repairs, this daily flow
averaged 75,407 gpd for the remainder of 2001. The primary source of supply for this
system consists of a series of three springs known as the Bonnie View Springs. Another
supply source owned by the Company and used historically is the Depot (Station) Road
Spring. Other sources of supply listed in WSA No. 5889 and historically used in this
system, but not owned by the Company are requested to be removed from the approval.
The Company has also proposed the use of two new wells: 1) "Pine Hill #1" a new well
with a sustained yield of 15 gallons per minute (gpm) and the existing "Station Road
Well" having a sustained yield of 39 gpm. The "Station Road Well" is not located
on
property owned by the Company, but it is proposed to be acquired through ownership
transfer or easement. These two wells are proposed to satisfy a regulatory requirement to
ensure sufficient supply to meet the "average day demand" with the primary source out of
service. The "average day demand" for this system has been calculated to be 52 gpm.
