Project Description:
The applicant proposes to renew and modify an existing permit to mine 75 acres of limestone from a 97.3 acre parcel owned by the applicant, 2500' east of County Route 47, 2000' northerly of the intersection of County Route 47 and County Route 143, approximately 2.5 miles south of the hamlet of Great Bend.  It is proposed to add 13.7 acres owned by by Kay M. Hadley-Dugas, 23073 County Route 47, Carthage, New York 13619, for stockpile area only, resulting in a new life of mine boundary of 88.7 acres.
State Environmental Quality Review (SEQR) Determination: Project is a Type I action and will not have a significant effect on the environment. A coordinated review with other agencies was performed and a Negative Declaration is on file.
SEQR Lead Agency: NYS Department of Environmental Conservation
State Historic Preservation Act (SHPA) Determination: Cultural resource lists and map have been checked. No registered, eligible or inventoried archaeological sites or historic structures were identified at the project location. No further review in accordance with SHPA is required.
Coastal Management: This project is not located in a Coastal Management area and is not subject to the Waterfront Revitalization and Coastal Resources Act.

Project Description:
The applicant in conjunction with the U.S.D.A. Natural Resources Conservation Service, Lowville, New York office proposes to develop approximately ten acres of waterfowl nesting pothole habitat in regulated freshwater wetland G-1, a 436 acre Class II wetland, predominately shrub swamp at project location.  The site is 2000' west of the hamlet of Pine Grove off Pine Grove Road, approximately 5.75 miles east of the Village of Lowville.
State Environmental Quality Review (SEQR) Determination: Project is a Type I action and will not have a significant effect on the environment. A coordinated review with other agencies was performed and a Negative Declaration is on file.
SEQR Lead Agency: NYS Department of Environmental Conservation
State Historic Preservation Act (SHPA) Determination: A cultural resources survey has been completed. Based on information provided in the survey report, the New York State Office of Parks, Recreation and Historic Preservation (OPRHP) has determined that the proposed activity will have no impact on registered or eligible archaeological sites or historic structures. No further review in accordance with SHPA is required.
Coastal Management: This project is not located in a Coastal Management area and is not subject to the Waterfront Revitalization and Coastal Resources Act.

This Department has received a request to issue a permit and has drafted an Article 19 (Air Pollution Control) of the Environmental Conservation Law, an Air State Facility Permit, at the facility located at Kendall Company, LP, Oriskany Falls, New York 13425. The facility manufactures hospital and medical products. The emission unit for this project is U-00010 and the emissions from this unit consist of particulates, volatile organic compounds, ozone, and carbon monoxide.   

This permit includes federally enforceable conditions to limit considered to "cap" volatile organic compounds emissions below the applicable threshold level set forth in 6NYCRR Part 212.10.  The draft permit with conditions is available for review in the Region 6 NYS DEC offices in Watertown and Utica, New York.
