Project Description:
The applicant proposes to realign and reconstruct approximately 2,700 feet of County Route 6 (CR6) from its intersection with Bailey Hill Road to the west for approximately ½ mile to improve road alignment.   The project will include approximately 750 feet of Mud Creek realignment, from the south side of CR6 between Leon Bridge No. 10 and Leon Bridge No. 9 to the north side of CR6 to allow elimination of Leon Bridge Nos. 9 and 10.  Heavy rock riprap streambank erosion protection will be installed along the beginning and end of the newly constructed channel; shrub willows will be planted at various locations along the newly created streambanks for natural erosion control and rock habitat improvement structures, such as v-shaped gravel traps, will be installed within the new stream channel.  Additionally, two aluminum box culverts, Leon Culvert Nos. 12 and 13, located approximately 245 feet and 775 feet from CR6 Bailey Hill Road intersection will be replaced with larger concrete box culverts and Leon Bridge No. 11, located approximately 1,025 feet from the intersection will be replaced by a 50 foot long concrete box beam bridge structure.
State Environmental Quality Review (SEQR) Determination: Project is a Type I action and will not have a significant effect on the environment. A coordinated review with other agencies was performed and a Negative Declaration is on file.
SEQR Lead Agency: Cattaraugus County Dept of Public Works
State Historic Preservation Act (SHPA) Determination: Cultural resource lists and map have been checked. No registered, eligible or inventoried archaeological sites or historic structures were identified at the project location. No further review in accordance with SHPA is required.
Coastal Management: This project is not located in a Coastal Management area and is not subject to the Waterfront Revitalization and Coastal Resources Act.
Opportunity for Public Comment: Comments on this project must be submitted in writing to the Contact Person no later than May 24 2002.

Project Description:
The Department has prepared a draft permit and made a tentative determination to approve this application for a modification of an existing permit to include the discharge of approximately 100 gallons per day collected from floor drains in the bus garage maintenance and storage areas and then run through an oil/water separator system and discharged to an unnamed tributary of Mill Creek.  The facility is located at the High & Middle School location, located on the west side of NY Route 60, approximately 0.5 miles north of the Village of Sinclairville.  The application and draft permit are available for review at the Buffalo Regional Office during regualr business hours.      
State Environmental Quality Review (SEQR) Determination: Project is an Unlisted Action and will not have a significant impact on the environment. A Negative Declaration is on file. A coordinated review was not performed.
SEQR Lead Agency: None Designated
State Historic Preservation Act (SHPA) Determination: The proposed activity is not subject to review in accordance with SHPA. The permit type is exempt or the activity is being reviewed in accordance with federal historic preservation regulations.
Coastal Management: This project is not located in a Coastal Management area and is not subject to the Waterfront Revitalization and Coastal Resources Act.
