Installation of a complete water supply and distribution system to serve Champion Water District No. 4 (NYS Route 26 and Cole Road) and take a supply of water in amounts to average 49,275 gpd for use in the system through a connection to the existing facilities of the Village of West Carthage.

The applicant proposes to reconstruct approximately 4.2 miles of NYS Route 12, between the City of Watertown and the Village of Clayton. The project requires the filling of 0.17 acres of regulated freshwater wetland CL-6, 1 140 acre Class II wetland associated with Three Mile Creek, and 0.10 acres of regulated freshwater wetland CL-3 an 18 acre Class III shrub swamp. Approximately 1.0 acres of constructed wetland will be developed in mitigation for wetland loss.

The applicant proposes to construct a 175' long x 16' wide corridor and gravel access road across regulated wetland B-41 to a proposed camp building site. The camp, 36' x 28', with septic system and parking area would be in the 100' buffer area of wetland B-41. Also, a 20' long x 16' wide timber bridge would be constructed over an unnamed tributary of Sylvia Lake on an existing roadway for access through property and a 24' x 30' "out camp" would be constructed along that roadway, primarily for day use. A 12" water equalization culvert would be installed in an existing right-of-way roadway to the west of the proposed roadway. The site is located approximately 0.5 miles southwest of the hamlet of Balmat.

The applicant proposes to mine 9.9 acres of sand and gravel from a 9.9 acre life of mine site on a 53.3 acre parcel owned by Benedikt Thrana, NYS Route 310, Madrid, NY 13660. The site is located 25' north of NYS Route 310, 2500' southwest of Ruddy Road, approximately 2.5 miles northeast of the hamlet of Madrid. Crushing and/or screening is proposed.
