The applicant proposes to abandon an existing well (8-2), and to install a new public supply well (8-2A) to a depth of approximately 550 feet with a pump capacity of 1110 gallons per minute. The well is to be used for public water supply purposes. The project is located at Plant 8, Franklin Avenue, Malverne, Town of Hempstead, Nassau County.

The applicant proposes to remove and replace in the same location 68 linear feet of bulkhead and add a 10 ft. return. Construct 12 linear feet of bulkhead to close off the existing earthen boat slip and place 50 cubic yards of upland fill. Dredge to a maximum depth of 4 ft. below mean low water no greater than 10 ft. seaward of the bulkhead with resultant 25 cubic yards of dredged material to be used as backfill. Applicant proposes to construct a 20" (casing) x 14" (screen) diameter production well (no. 3) to a depth of approximately 180 feet to provide an additional 1300 gallons of water per minute to the Port Jefferson-Farmingville High III water supply system which is located within the Patchogue District of the SCWA. The project site is located on the east side of Fairmount Avenue, 520.45' north of Horseblock Road, Medford, Town of Brookhaven, County of Suffolk.

Applicant proposes to demolish existing tollgate, toll building, and concrete posts, and construct a 3,275 square foot facility operations office, attached garage, new toll plaza, sidewalk, and maintenance vehicle access. All construction will take place within a 1.6 acre area of the existing Smith Point parking lot. The project is located in Smith Point County Park, T - Brookhaven, Suffolk County.

The applicant proposes to construct a single family dwelling, deck, sanitary system, driveway, drywells and place 210.7 cubic yards of fill within the 100 ft. adjacent area of freshwater wetland M-45. The parcel is located at Tulip Court, Moriches, Town of Brookhaven, County of Suffolk, SCTM No. 0200-856-2-9.1.

Applicant proposes to construct a single family dwelling and associated septic system and driveway within 60 feet of NYSDEC freshwater wetland # B-11. The project is located on Moss Creek Lane, Patchogue, T - Brookhaven, Suffolk County.

The applicant proposes to install two public supply wells at a new Plant No. 11. Each well will be installed to a depth of 600 feet and have a pump capacity of 1380 gallons per minute. The project is located at the northeast corner of Dix Hills Town Park, south of Northwood Court, Town of Huntington, Suffolk County.

Applicant proposes to install a 7' x 13' launching ramp for public boat access consisting of a 6" deep bed of 3/4" crushed native stone on filter cloth which will be contained by geolok confinement cells.
