Project Description:
 The applicant proposes the mining of sand and gravel in two phases on a cumulative total of 324 acres out of approximately 700 acres owned (by the applicant) and has applied for permits required for Phase 1. Separate permit applications and a Supplemental Environmental Impact Statement will be submitted for Phase 2. Phase 1 involves mining above and below the water table and material processing with a wash plant with a closed-cycle process-water circulation system, affecting a total of about 201.2 acres. Phase 1 includes constructing the main haul road to access Cattaraugus County Route 80 (Laidlaw Road), the installation of a bridge crossing of Johnson Creek, culvert crossings of two tributary streams, the construction of a dam for Mining Area 1 north of Johnson Creek and the mining of 0.28 acres of Federally regulated wetlands. The reclamation objectives for Phase 1 alone is five lakes totalling about 66.3 acres, 49 acres of prime agricultural land and the remainder as wild life habitat. As an alternative for Mine Area 9 along Laidlaw Road, this area may be the first area mined and then reclaimed suitable for a County park with a lake of about 8.5 acres. The estimated duration of Phase 1 is 22 to 24 years. The proposed Farmersville Mine Operation site is located north and south of NYS Route 98 and east, west and south of Laidlaw Road.

Project Description:
 The applicant proposes to install two 50 lineal ft. sections of broken concrete riprap along Conewango Creek to protect the streambanks from erosion at an existing foot bridge. A rock grade control sill will also be installed across the stream at the downstream section of the proposed riprap. Project Description:
 The applicant is a stand alone dry cleaner with two 3rd generation perchlorethylene (perc) machines, two petroleum washers and four petroleum dryers. The facility is located at 316 West State Street. The current usage of perc is 8000 gallons per year and there is approximately 26 tons per year (tpy) of Volatile Organic Compound (VOC) emissions, making it subject to the provisions of 6 NYCRR Part 232 for the existing emissions. The permit will contain federally enforceable permit conditions limiting VOC emissions to 49 tpy, limiting total hazardous air pollutant emissions to 24 tpy and limiting the emission of perc to 9.5 tpy. The application and draft permit are available for review at the Region 9 office in Buffalo.

Project Description:
 The Department has received an application from the company's Mayville Division, a manufacturer of wooden residential furniture, a major source, under Title V of the Clean Air Act Amendments since its actual emissions of volatile organic compounds (VOCs) is greater than the threshold value of 50 tons per year and the unrestricted potential to emit for total of hazardous air pollutants exceeds the major source thresholds. Surface coatings used are compliant with the requirements of VOC reasonably available control technology of 6 NYCRR 228. The facility also complies with the requirements of the National Emission Standards for Hazardous Air Pollutants for Wood Furniture Manufacturing Operations, 40 CFR Part 63 Subpart JJ by keeping the monthly pounds of volatile hazardous air pollutants per pounds solids consumed ratio below 1.0. The application and draft permit are available for review at the regional office, and written comments will be accepted. If comments raise significant issues the department may hold a public hearing or modify the draft permit. In accordance with 6 NYCRR Parts 621.5(d)(9) and 201-6.4(c)(1), the US Environmental Protection Agency (USEPA) has the authority to bar issuance of any Title V Facility Permit if it is determined not to be in compli- ance with the Clean Air Act or 6 NYCRR Part 201. After the comment period, EPA will have an additional 45 days to review the draft or modified draft permit. If the US EPA does not object in that time, the Department may issue the proposed permit as the Title V permit.

Project Description:
 The applicant proposes to remove accumulated sediment from an approximate 180 ft. long x 80 - 100 ft. wide section of the inlet area of "Deerwood Lake", a 7.2 acre pond located within the course of tributary 16 of Wiscoy Creek. The water level of the pond will be lowered four feet to accomplish sediment removal. Removed sediment will be used for landscaping areas west and east of the pond. The project site is at the applicant's property, Telegraph Road, approximately 2,500 feet south of the intersection of Telegraph and McElroy Roads.
