Project Description:

The applicant proposes to operate a pet crematory facility at the site of an existing pet cemetery located at 760 Middle Country Rd., Middle Island, NY, (SCTM# 200-402-01-24.7). This pet crematory replaces an existing facility at the site which was destroyed by fire on July 2, 2010. The facility has an Air State Facility Permit to operate five(5) crematory units which were all destroyed by fire. The modified permit will include the construction of five (5) replacement crematory units, which when combined with the two existing units will provide a total of seven (7) units. All units will be located inside the building which has already been reconstructed. The total oxides of nitrogen (NOx) emission from the facility will remain under 22.5 tons per year. A Solid Waste Management Facility Permit will be issued for the operation of a pet crematory with a maximum design capacity of 8,120 pounds per day of animal carcasses and remains. No infectious waste will be received or processed. The project is not within the NYSDEC regulated Wild, Scenic and Recreational River areas, or Freshwater Wetlands which are associated with the Carmens River.

State Environmental Quality Review (SEQR) Determination:

Project is an Unlisted Action and will not have a significant impact on the environment. A Negative Declaration is on file. A coordinated review was not performed.

SEQR Lead Agency: None Designated

State Historic Preservation Act (SHPA) Determination:

A Structural-Archaeological Assessment Form has been completed. The proposed activity will not impact on registered, eligible or inventoried archaeological sites or historic structures.

Coastal Management:

This project is not located in a Coastal Management area and is not subject to the Waterfront Revitalization and Coastal Resources Act.

Opportunity for Public Comment:

Project Description:

The Department has made a tentative determination to approve a permit modification to upgrade an existing laundromat wastewater treatment system to a Generation 5 Aquarelle Filtration System. The discharge is to groundwater at the applicant's Trax Laundromat facility located at 90 Railroad Avenue, Ronkonkoma, NY. The proposed discharge is located within an area designated as a sole source aquifer. The following water purveyor has been identified as having a service area located within a 3-mile radius of the applicant: Suffolk County Water Authority.
