Project Title: Verizon Wireless Telecommunications Antenna Replacement-Crown Plaza

Location: 1 Olympic Drive in Lake Placid, Town of Elba, New York.

Project Description: The project involves the removal of two existing omni-directional antennas currently mounted on the roof of the Lake Placid Crowne Plaza Hotel and Golf Club and their replacement with a twelve-panel antenna array housed inside a stealth cupola structure to be constructed on the roof of the hotel. The cupola will be designed to blend with the building façade. Any exposed antennas, cables, and appurtenances will be painted to blend with the building façade.

The project also involves the installation of a 10 foot by 20 foot equipment shelter at the ground level within an 18 foot by 28 foot leased parcel and the installation of a 40 KW generator on a 4.5 foot by 9 foot concrete pad within a separate 6 foot by 12 foot leased parcel. Both the equipment shelter and generator will be behind (to the west of) the hotel building. Easements will provide for cable and utility connections between the cupola, equipment shelter, and generator.

The omni-antennas to be removed reach a height of 53 feet and 63.5 feet above the finished floor. The panel antennas to be installed reach a height of 52 feet above the finished floor. The cupola to be constructed reaches a height of 56 feet above the finished floor, has a footprint of 14 ± feet by 14 ± feet (196 ± square feet), and will be built of radio-frequency transparent materials.

Project Description: A three lot subdivision of tax parcel 160.5-1-3 and a two lot subdivision of tax parcel 154.17-1-4. Tax parcel 4 will be subdivided to provide a suitable on-site waste water treatment system area for a proposed new single family dwelling on parcel 3. Tax parcel 3 will be divided into three lots: a 0.38 ± acre parcel will be conveyed to the owner of tax parcel 4 for a new driveway (an existing driveway to tax parcel 4 through Lands owned by the State of New York will be abandoned), a 4.98 ± acre parcel is proposed for purposes of constructing one single family dwelling with detached garage (this proposed lot will receive the 0.38 ± acres proposed to be subdivided from tax parcel 4), and a third lot, 2 ± acres in size and also on tax parcel 3, is proposed to be improved by a single family dwelling that was approved by Agency permit 2008-293.

Project Title: Independent Towers Holdings, Inc.-Chester Cell Tower

Location: Off Old Schroon Road in the Town of Chester, New York.

Project Description: The project as proposed is a subdivision into sites involving the lease of a 100 foot x 100 foot parcel for the construction of a new 90 foot tall above existing ground level telecommunications tower within a 75 foot x 75 foot fenced-in equipment compound. The compound is proposed to be located westerly of the existing Niagara Mohawk utility right-of-way. Access to the tower will be by a 12 foot access drive from Old Schroon Road; the drive will be within a 20 foot utility easement for underground utilities. The tower will be a simulated tree (white pine) with branching extending from 51 to 91 feet above ground level. Within the fenced compound AT&T Mobility will install an 11.5 foot by 20 foot equipment shelter and generator. The lease agreement, among other things, also protects the existing trees within 300 feet of the proposed tower. The project has received all necessary approvals from the Town of Chester.
