Project Description:

The applicant proposes to reopen a previously permitted sand and gravel mine located west of the intersection of CR 78 and CR 79, and north of CR 79. The site is further located approximately three miles north of the hamlet of Woodville, four miles south of the Village of Henderson. The previously approved mine was for 15 acres, the current proposal is 9.2 acres. The site has been extensively disturbed.

Project Description:

The Department has made a tentative determination to issue the permit for a State Pollutant Discharge Elimination System permit under Article 17 of the Environmental Conservation Law. The approved application is for a modification of a permit for an existing discharge of 0.055 MGD (million gallons per day) of treated sanitary wastewater into the Black River, a class C waterbody from a secondary treatment plant located at the applicant's facility in the Village of Port Leyden, Lewis County. Sampling requirements for BOD and TSS are being increased from monthly to twice monthly.
This tentative determination indicates that a discharge regulated under this statute is considered to satisfy regulatory standards for permit issuance, based on information available to the Department at this time. As a tentative determination, the Department seeks comments on the proposed regulated activity prior to making a final decision on permit issuance. As such, this tentative determination does not signify department approval for or endorsement of the activity for which a permit is sought or for the overall proposed project. Project Description:

A Part 360 Permit was issued to St. Luke's Hospital on September 26, 2007.The permit authorized the facility to operate their two on-site autoclave units which treat red bag medical waste from the hospital and the surrounding community. The issued permit require new validation testing for each unit and required effective treatment of RMW (regulated medical waste ) by demonstrating a 4 log10 reduction in "G Stearothermophilus" spore bacteria.
The facility had to modify their existing Aduromed autoclave units by adding new vacuum pumps and controls. On November 12, 2008 a complete validation test was conducted on each autoclave unit and the results proved sufficient in meeting permit conditions. The issued Part 360 Permit will now be modified to reflect minimum operational requirements of the autoclaves. Each unit must be operated for a minimum treatment cycle of at least 60 minutes at 275 degrees F and include 3 precondition vacuum phases.

Cultural resource lists and map have been checked. No registered, eligible or inventoried archaeological sites or historic structures were identified at the project location. No further review in accordance with SHPA is required.

This project is not located in a Coastal Management area and is not subject to the Waterfront Revitalization and Coastal Resources Act.

Opportunity for Public Comment:
