Project Description:

The applicant proposes to introduce Triploid Grass carp to Sylvan Lake (NYS water index # H-95-14-P352, Class Bt) as a measure to control invasive weeds, specifically Eurasion Milfoil. The permimeter of the Lake is part of State regulated freshwater wetland HJ-13, a class 2 wetland. The applicant will also be required to construct a barrier to prevent the migration of grass carp from the lake and conducting appropriate inspection and maintenance of the barrier. The appropriate amount of grass carp introduced to the lake will be determined by NYSDEC fisheries staff and based upon vegetated acreage within the lake. Triploid grass carp may only be purchased from approved suppliers possessing a valid permit to import and sell triploid grass carp in New York State.

Project Description:

The City of Middletown is requesting a permit to reconstruct the Mill Pond Dam on the Shawangunk Kill (Water Index # H-139-13-19, Class B) near the intersection of County Routes 24 and 94 in the Town of Mount Hope, Orange County. The dam has been owned by the City of Middletown since 1998 and the pond serves as a water supply impoundment for the City. The total project site is 0.85 ± acres and the proposed work will impact approximately 0.25± acres.
The proposed reconstruction includes renovations to the concrete spillway, replacement of a 12-inch diameter outlet pipe and valve box and construction and minor regrading of a new access way/driveway. The existing dam is 110 feet long with a 75-foot long spillway centered between two non-overflow sections. The entire length of the dam will be reconstructed. The applicant proposes to angle the existing spillway and enlarge it to 133 feet in length while generally maintaining the height of the spillway and raising the height of the non-overflow sections by approximately 4 feet above the spillway so that these sections are not overtopped during 100-year flood conditions.
During construction, a temporary drawdown may be required. It is anticipated that construction will be completed within 6 to 12 months. No dredging is proposed except minor excavation required to facilitate the proposed construction.
This project will allow the City of Middletown to skim the top 2.5 feet of water from the pond and pump it into an existing 24-inch raw water main that discharges into the City-owned Shawangunk Reservoir. A portable pump will be used until the permanent pump station is designed and constructed. The project will also allow the applicant to address existing structural deficiencies in the dam and enable compliance with the NYS Department of Environmental Conservation's hydraulic and structural stability criteria.

The applicant proposes to discharge to groundwaters using a subsurface absorption bed system up to 5,000 gallons per day of treated (on site treatment facility) sanitary wastewater from a proposed automobile dealership, ~22,720 square feet(sf) and a proposed supermarket, ~ 56,430 sf to be located on ~16.4 acres of a 47.5 acre parcel. The proposed development is located on NYS Route 94 east of Sanfordville Road. The applicant has proposed to monitor several effluent parameters including BOD, coliform and total suspended solids on a monthly basis to document treatment plant performance.

Project Description:

The applicant, Greenfield Meadows, Inc, has applied for permits to expand the existing Greenfield Meadows Bungalow Colony by constructing 39 three-bedroom residential units, in addition to the 8 existing three-bedroom units, and to replace the existing subsurface sanitary discharge with a surface sanitary discharge of approximately 19,000 gallons per day to the West Branch of the Beer Kill (Index # H-139-14-38-3-1, Class B-TS). Based on the Department's reviewed of a Waste Assimilative Capacity (WAC) analysis submitted by the applicant's engineer, the Department has prepared a Draft SPDES permit (SPDES# NY-0274003) and made a tentative determination to approve this discharge of treated sanitary waste. The applicant also proposes to construct two outfall structures on that same stream, the West Branch Beerkill, for the discharge of the treated sanitary effluent and a separate discharge for stormwater. The Department staff has reviewe and approved a Stormwater SPDES Pollution Prevention Plan for the collection and discharge of stormwater. Greenfield Meadows is located on 43 acres adjacent to the north side of Tamarack Road, off NYS Rt. 52 and approximately 5.7 miles west of Ellenville, in the Town of Wawarsing, Ulster County, NY.

The applicant proposes to construct a new single-family residence located partially within the 100 foot adjacent area of Freshwater Wetland O-9 (Class I), including grading, construction of a new water supply well and two dry wells, new retaining wall, rip-rap outlet protection for a stormwater outfall and excavation of a trench for installation of a water supply line. No disturbance to the wetland is proposed; the total area of disturbance to the adjacent area of the wetland is 0.13-acre, and includes revegetation of disturbed areas.
