Forms for Attending Camp
Some of these forms must be filled-out correctly and signed or the camper cannot remain at camp!
Forms marked with an "*" are required upon arrival at camp. If they are not filled out and signed, the child cannot remain at camp.
Required Forms:
Applications: Can be filled-out online, then printed and mailed with a check or money order. Applications must have a postmark no earlier than February 11th for organization sponsored children and a postmark no earlier than March 1st for parent-paid children.

- Application for 15-17-year olds to Pack Forest Teen Ecology Workshop (157 KB, pdf)
*Medical Form, required upon arrival at camp: Cannot be filled-out online. Must be printed and filled-out by hand by parent/guardian and health care provider before the child arrives at camp.

Additional Forms:

- Permission to Shoot Form (377.9 KB, pdf): Needed to take Hunter Education (Hunter Safety) classes at Camps Colby, DeBruce and Rushford or to participate in Sporting Sports at Pack Forest
- Model Consent Form: An optional form necessary for DEC to use photographs taken at camp in promotional materials
